    Detect and Prevent Harm from Medications

    Adapted from proven adult trigger tools, the Pediatric ADE Trigger Tool provides a powerful yet simple method to detect medication-related harm in pediatric inpatients. The related Trigger Toolkit available below can help hospitals implement medication system changes to ensure fewer drug-related injuries.

    The toolkit includes triggers CHCA hospitals have found most useful during chart reviews to identify ADEs, measure the level of harm from each ADE and identify areas for improvement.

    Neonatal Intensive Care Unit (NICU) Trigger Tool
    Detect and Prevent Adverse Events in Fragile Neonates

    The NICU Trigger Tool is the nation’s first diagnostic tool to identify medication and non-medication related adverse events in the neonatal intensive care unit (NICU). This tool will help children’s hospitals identify and apply improvement methods that proactively address adverse events within this fragile patient population before harm occurs. A collaborative effort between CHCA and the Vermont Oxford Network (VON), the NICU Trigger Toolkit includes a list of potential triggers and instructions for collecting the data needed to measure the rate of adverse events (AEs) and adverse drug events (ADEs) in the NICU.
